The link below allows you to download the form. There are several options for completing and returning the form. Blank paper forms will be available at camp if you prefer to use those.

After downloading the form, you may either print the form and fill it in by hand or type your responses into the form.

If you fill in the form by hand, bring it to camp with you when you come.

If you type your responses into the form, save it to your personal files and email it to troopmitre@gmail.com or print it and bring it to camp with you.
Note: if you save the completed form in your files, for the next event, you can save time by opening that file and updating it rather than completing a new form.
